Appointments

Routine eye exams are available by appointment from 8:30 - 4:15 Monday, Wednesday and Thursday, from 9:00 to 4:15 on Tuesday and from 8:30 to 1:00 on Friday. To schedule an appointment, please call 817-431-2020 during our regular office hours. We realize at times it may be necessary to cancel or reschedule, if you are unable to keep your appointment we ask that you give at least 24 hours notice so we may be able to offer this time to another patient. For last minute cancellations and no shows fees may be assessed.

If you are late for your appointment, you may be asked to reschedule. We do understand that your time is also valuable. We will make every effort to keep your wait to a minimum, however emergencies may cause unforeseen delays.

Urgent Care

Eye infections and injuries are worked in throughout business hours. For after hours medical emergencies please call 817-431-2020 and you will be instructed on how to reach a doctor. Please be aware there are additional fees for after hours medical visits.

Payments and Insurance

Insurances accepted are: Aetna, Avesis, Blue Cross Blue Shield, Cigna, Davis Vision, Eyemed, Humana Comp Benefits (VCP), Medicare, Opticare Vision Plan, Superior Vision, Vision Benefits of America and Vision Service Plan.

Payment is due at the time of service. We accept payment by cash, check, Visa, Mastercard and Discover.

Office Policies

• There will be a $25 fee for appointments missed or not cancelled within 24 hours.
• All co-pays and payments are due at the time of service.
• ID and insurance card must be presented at time of appointment.
• If insurance information is not made available at the time of visit, we will not be able to file for you.
• We require your insurance information be available at the time of scheduling your appointment.

Patient Forms and Feedback

We have provided our patient questionnaire as a PDF file you can print and complete. In addition, we've provided a feedback form so that you can let us know how we're doing. This will only take a couple of minutes to complete and we'd appreciate your comments so that we can better serve you.
